Referral information for GPs/clinicians only

All these services are open access, but we welcome referrals by GPs and health or social care workers so that those who would most benefit are helped to get in touch. It also helps to have clear outcomes agreed with the patient - improving diet, doing more exercise, self-esteem, looking out for falls risks, concerns about housing or winter warmth.

We will carry out a holistic assessment with the client and refer them to services available in the community, whether ours or from other organisations. Our advice team are well informed about matters that affect the wellbeing of people in later life.

Referral forms are available from our web site, but are not essential.
